As a tenant Admin I want a field on the form to capture whether the booking involves more than one department.

Please add the Y/N question "Is this a collaboration between two or more departments?" to the form under "Booking Type"

If yes, then the department menus pop up
Is it possible for them to select multiple departments if necessary?
