This is looking like a great template Mikala!

A couple of thoughts:

• I think a heading before "Out of date or missing content" that just provides a link to the page would help immediately orient us.

E.g.

• Should "Decision" be at the bottom?
• I have started a PR (Add issue template for removing content #423) to propose removal of a page. Do you think we should link to that if the decision is to remove?

That PR (#423) is in the yaml format for issue templates. I think once we decide here what the "Update" template should look like we can open a PR with the yaml format for the "Update" template.
