Table breaks across pages

We definitely don't want tables breaking across pages, as on pages 4-5, except for extreme pagination issues (like Table 2.11 requires) or tables that are too large for a single page. Can that be part of the default markup for tables? (I haven't tried to look at the source yet, and will need to educate myself on the new format.)

We should be able to use the fo, but that does not appear to work.
