how to generate Rag_result with 'query_id', 'query','gt_answer', 'response'

In the document we see that RAGChecker requires two inputs one is rag_results and all_metrics. How to generate rag_results for the own data. I want to evaluate my rag application on large set of user query.Please help me with the step by step process.
