docs: update CI review for server-side architecture

Updates the CI code review documentation to reflect the new server-side architecture (PR #5581 on elementary-internal).

Key changes:

• No more Anthropic API key -- the review runs server-side via Bedrock, customers only need their Elementary API key
• Simplified setup -- one secret instead of two
• Removed obsolete options -- claude-model, models-path, diff-filter, base-ref, mcp-config-path no longer apply
• Added env-id docs -- for repos connected to multiple environments
• Updated review sections -- now covers change analysis, performance/cost (SQL anti-patterns), downstream impact, tests/incidents
• Updated troubleshooting -- new error messages for repository not connected, multiple environments
• Added issues: write to required GitHub permissions (needed for posting PR comments)
