Refactor Middleware

Eliminated the standalone middleware transformer functions (inbound.ValidatorMiddleware and outbound.Middleware) and replaced them with Middleware() methods implemented directly on the concrete validator/provider structs.

What changed

Introduced two embeddable base structs that carry the middleware logic directly:

inbound.ValidatorBase (new):

Embeddable in any TokenValidator concrete type
Provides Middleware(apiKeyHeader string) func(http.Handler) http.Handler directly on the struct
Wired via v.ValidatorBase = inbound.NewValidatorBase(v) in constructors
Replaces the standalone inbound.ValidatorMiddleware(v, header) transformer

outbound.ProviderBase (new):

Embeddable in any TokenProvider concrete type
Provides Middleware() func(http.Handler) http.Handler directly on the struct
Wired via p.ProviderBase = outbound.NewProviderBase(p) in constructors
Replaces the standalone outbound.Middleware(p) transformer

Each init() factory now calls v.Middleware(header) / p.Middleware() instead of wrapping through an external adapter. This matches the pattern already used by transform.CompiledTransforms.RequestMiddleware().
