refactor: nest tools under single parent, use unpinned/pinned/disabled, rich labels

- All tool groups nested under one 'tools:' collapsible parent
- Dropdown states: unpinned (enabled), pinned (enabled + always visible),
  disabled — makes clear that pinned is also enabled
- Translations now include: friendly name, (read-only)/(destructive) hint,
  [mandatory] tag, and brief description — all auto-generated from
  tool source code via extract_tools.py
- Mandatory tools (ha_search_entities, ha_get_overview, ha_get_state,
  ha_report_issue) clearly labeled in the UI
