Tri-linkage discipline rollout (doc ↔ bead ↔ GH-issue)

Umbrella issue for AC-12 enforcement per plan 027 § 3.5 PR-1 + § 5.5.

Scope

Backfill mandatory Beads: front-matter row in every 000-docs/*.md file across the 5 IEP repos that is touched by Skill Refiner work. Activate validate-trilink.sh CI gate per repo, label-filtered to refiner (legacy beads exempt — plan § 11).

Verifier

intent-eval-platform/scripts/validate-trilink.sh (authored 2026-05-27 alongside Step 1).

Three checks:

1. MISS-DOC / MISS-GH — every refiner-labeled bead carries Doc: + GitHub: lines
2. MISS-FRONT — every 000-docs/*.md citing bd_000-projects-* carries a Beads: front-matter row
3. MISS-GH-BEAD / MISS-GH-DOC — every refiner-labeled GH issue carries Bead: + Doc: lines

Exit non-zero on any violation.
