Add detailed tracking and display of the instruction proposal phase.

Tasks:

1. Create an InstructionProposalTracker class in the telemetry module
2. Modify the DSPy integration to capture:

• Candidate prompt instructions from the proposer model
• Time taken for each proposal
• Progress indicators (percentage, loading bar)

3. Implement real-time display of progress during optimization
