pdf attachments for anthropic (&other supporting providers)

Anthropic supports pdf ingestion. This isn't widely supported between providers, though I believe gemini does.

patches codecompanion to add /attachment tool which allows attaching a pdf to a code companion context for supported adapters. It documents how to add support for a other providers.
