Core bug: `fetch` usage does not have a timeout

The fetch usage right now in Node v18 does not timeout properly, it hangs indefinitely when a bad lookup is performed.

@twiss we made a PR at #3 but it wasn't encouraging when it wasn't merged after 2 years.
