Complete the simulation of the second VAX 8200 (then VAX 8300) CPU

There already is a stub of the second CPU (named KA1) in the VAX 8200 simulator, but it does not work as of yet. The SHOW CPU command does not work; according to the manual is should show:
$ SHOW CPU
Attached processor is in the EXECUTE state.
but it shows:
$ SHOW CPU
%SYSTEM-F-DEVOFFLINE, device is not in configuration or not available

The second CPU is activated in the ini file: "SET KA1 ENA"
The MP license is installed: "Installation of VMSMPS V4.4 completed at 16:51"

One underlying problem is that most SIMH/ZIMH infrastructure uses global variables to store state, like processor registers. This means that having two instances of a processor is difficult. However, part of my aim is to fix that.
