docs: premium cap as % of credits

Update model-categories docs and the 2026-06-10 changelog entry to
reflect the premium weekly fair-use cap now being 10%/15%/20% of monthly
credits (Lite/Pro/Max) instead of fixed 10/50/140 credit amounts.
