✨ feat(app): ship Token Menu Bar

Make provider limits, resets, credits, and local usage available from the
menu bar without switching between provider dashboards. Keep account
selection and retained history separate so changing credentials cannot
mix another account's totals into the current view.

Keep provider parsing, scheduling, storage, and presentation policy in
Core. Use a screen-bounded macOS panel with persistent tab content and
on-demand supporting detail. Preserve reported zero and unknown values,
and keep credential reads noninteractive.

Cover macOS 14, 15, 26, and 27 with runtime-checked CI. Isolate verification
from the installed app, credentials, network, and persistent preferences.
