Documentation of Provider Attribution

There is no documentation of the Provider Attribution input layer table. Please put up some information on Docs that review:

• The required input layer data model
• The description of the column
• The intended use of this table

It would also be great if we could refactor the code to accept enrollment at the span level since enrollment can be at this grain.
