Refine meeting-summarizer classification and composition rules

Summary

Six targeted refinements to the meeting-summarizer skill in response to a careful diff between the skill's output and a hand-refined summary. Each change tightens an ambiguous rule or adds a missing rule surfaced by real usage.

Changes

F1	Meeting title	Prefer a content-derived title; use the calendar name only when no theme emerges
F2	Decisions	Combine tightly coupled decisions (one is the mechanism/rationale of the other) into a single bullet
F3	Open Questions	Recognize implicit deferrals ("we haven't decided yet", "either X or Y", "for now ... but we might ...") in addition to explicit parking
F4	Attendees	Speakers only — drop silent invitees from the calendar invite
F5	Discussion Points	"One topic per heading" does not mean "one bullet per heading"; group related subtopics under one heading
F6	Compression	Preserve stated constraints ("only X", "not via Y") verbatim; they are the highest-value compression

Each change also adds corresponding entries to the Rationalizations, Red Flags, and Common Mistakes tables for discipline enforcement, and the template comments were updated to match.
