This depends on the changes in PETL-133, and is an initial POC for moving away from Pentaho and to a SQL-only approach. This isn't really ready to merge, as it still involves properly plugging this in and disabling the existing mw_patient job, but it is ready enough to run and compare with the existing table and see how things look for accuracy and performance.
