fix(tui): simplify console org display

Summary

• remove the console org icon and org suffix from prompt and model/provider labels so provider information only appears once
• keep the connect dialog indicator as a simple connected checkmark while showing the active org name in the row footer
• remove the home-screen org hint so the org is no longer shown there

• repo-wide pre-push checks in a clean worktree before pushing
