New controlled terminology vignette

Required for new VLM workflow explanation. Will write an accompanying one for the corresponding new features in metatools and then update with a link. This fist draft covers both standard use for single CT and VLM.

Created as a branch off of @152-get-ct-vlm which closes #152 so the Pull Request #153 should be reviewed and merged prior to this one.
