Added template for new issues.

@tarlepp This is not a bad idea, but I guess you should start this PR on atoum/atoum, this is more relevant there since there is more contribution. @jubianchi thoughts?

I didn't make it there because I was working on other issues on this repo - so I just though first to see if this is accepted here and then make similar PR's to other repos also.

@Hywan templates are good in any repository of the org I think.

We have to work on a common template that will be added everywhere. We can start here and move to other repositories later.

As @Hywan and @jubianchi, i think issue template is a good idea, but i feel this one is too simplistically.

Without any deep reflexion, i think we need more information about versions used (atoum and dependencies, php), OS, ...
