chore(overwatch): expose deployed release metadata

What changed

• Exposes the deployed application version, Git revision, and Git ref from the existing deployment environment on /health.

Why

• Makes the running release independently observable so Overwatch can correlate a GitHub merge with the exact production revision.

Validation

• git diff --check
• The fields use the same environment variables already attached to OpenTelemetry resource metadata.
