Add agent diagnostics, MCP inspection, and safe launch mode

Adds read-only coding-agent diagnostics, secret-safe MCP configuration inspection, explicit safe and auto launcher profiles, and an agent-focused Omarchy skill guide with CLI-driven drift validation.

Highlights:

• omarchy agent doctor with structured JSON output
• omarchy agent mcp list and doctor without emitting config values
• safe and auto approval profiles for agent and prompt launches
• package-owned skill, shared-instruction, MCP, and transactional-edit guidance
• focused shell tests and skill route/link validation

Validation:

• focused agent doctor, MCP, launcher, skill, metadata, and bin-style tests pass
• command metadata check passes for 439 commands
• full non-graphical suite ran 210 shell test files; remaining failures require missing omarchy-pkgs fixtures or host capabilities such as writable runtime directories and network access
