TOPO model v5

This release comprises the latest model v5 which has the sigmoid removed and namespaces added.
See the readme for details on the versions.

This is the final model proposed for the L1 Menu v1.0 in 2026 in here: https://its.cern.ch/jira/browse/CMSLITDPG-1504
