feat(gnoweb): make heading text clickable to set URL hash

Still WIP

Wrap heading content inside the anchor link so clicking any heading updates window.location.hash, making section links shareable. Previously the anchor was an empty hidden element after the text.

• Move to wrap heading text (enter/exit)
• Replace aria-hidden with aria-label for accessibility
• Add focus-visible outline for keyboard navigation
• Add heading-anchor override in c-doc-view to prevent link styling
• Rebuild CSS
