Updated the Claimant Model docs for readability. There are no functional changes to the core model in here, but a lot has been reworded for clarity and quick lookup. The main changes in content are around witnessing. At the time of first writing, witnessing wasn't well defined. Now we have a much broader consensus and specs around what witnessing is. I've thus included them in the body of the Logs.md writeup, and adjusted the diagram to explicitly include witnesses as an active entity.
