Nomad Docs: Add Nomad State Metrics to Community Tools

Description

Add Nomad State Metrics to tools/community list.
A separate PR will add the project to Integrations.

I also:

• Removed the HC Levant entry since it is no longer supported
• Updated the Nomad Pack URL to be the docs page instead of the repo
