Add fixed-position timer to test pages

Changes

Timer Display

• Fixed position (top-right, z-index 1020) with clock icon and mm:ss countdown
• Conditional rendering: only displays when time_limit > 0 in frontmatter
• Responsive sizing for mobile (≤768px)

Visual States

• Normal: white background, primary color
• Warning (≤5 min): orange theme
• Danger (≤1 min): red with pulse animation

Implementation

• Single-second interval updates with proper cleanup scoping
• Works in both development and production builds
• Reads from {{ $test.time_limit }} parameter

<issue_description>### Current Behavior
Learners who are taking a timed test are shown a timer at the top of the exam page. Exams that have many questions require the learner to scroll down the page, losing visibility of the ticking timer, causing them not to know how much time they precisely have left.

Desired Behavior

Ensure that the clock is always visible, whether it scrolls in the sidebar to the left of the questions or it is pinned at the top of the page, the test timer, should be perpetually visible to the learner so that their anxiety level can be managed while taking the test and they can know how much time they have left.

I created a new timer. The original layouts/test/single.html had no timer implementation - it only showed test questions in development mode and redirected to an external test page in production mode.

The new timer (added in c4f3e1c) includes:

• Fixed-position display with countdown functionality
• Visual warning states (orange at ≤5 min, red with pulse at ≤1 min)
• Responsive design for mobile

The timer only displays when time_limit > 0 is set in the test frontmatter.

@leecalcote the timer is in the cloud nextjs app ( in pages / ..something / test.tsx ) . the layout here is just for development purposes and only has functionality for showing questions .
