Documents the Inject MCP Name Headers parameter for the MCP Support policy, adding a "Configuring Policy Parameters" section.

Split out from #1140 (MCP Support + MCP Schema Validation) so each policy is reviewed independently.

Rename the parameter to Inject MCP Element Headers and document the new
x-ms-mcp-element-id / x-ms-mcp-element-type header pair that replaces the
per-method x-ms-mcp-tool-name / -prompt-name / -resource-uri headers.

Reflects mulesoft-emu/microgateway-hybrid-policies#743 (mcp-support 1.1.1).
